Pokemon Heartgold comes in 3 basic configurations. Cartridge Only, Game In Case, and CIB. Game in Case: Includes the game in the original plastic DS click base. CIB: Includes Game, Clickcase, Paperwork, Outer Cardboard Box, Pokewalker. Generally these will be photographed as individual units, so you can see the one you are getting.
